WORKS

NorthStar Compliance Services is appointment-only. All appointments are scheduled by phone. This allows us to confirm the right collection type, prepare the correct paperwork, and ensure that a certified collector is available when you arrive.

Call to schedule
Call (435) 261-2088 to book. We will ask a few quick questions about the type of collection you need and who is requiring it, then put you on the calendar and tell you what to bring.
Same-day appointments are often available. If you need a collection today, call and ask. We will tell you honestly what we can do.

Please do not stop by without an appointment
Our collectors spend much of the week out of the office performing on-site and mobile collections across Sanpete, Sevier, and Juab counties. Our posted hours are the times we are reachable and scheduling, not a guarantee that someone is at the Ephraim office.
If you arrive without an appointment, there may be no one there to help you. Please call ahead.

What to have ready when you call
Please have the following items on hand when you call:
The type of collection you need, such as DOT drug, non-DOT drug, breath alcohol, or DNA. Whether the collection is required by an employer, a court, an attorney, or an agency. Any paperwork, chain-of-custody form, or donor packet you were given. The name and contact information for whoever is receiving the results. Your preferred day and time, and the address if you need us to come to you.

If you are not sure which service applies to your situation, call anyway. Figuring that out is part of what we do.

On-site and mobile collections
We travel to workplaces and other locations throughout our three-county service area. Mobile and on-site collections are quoted individually, since fees vary by location and group size. Call with your address and the number of donors and we will put together a quote and a date.

After-hours, weekends, and holidays
After-hours collections are available by appointment. Additional fees may apply. Call (435) 261-2088 to arrange one.

Employers and recurring testing
If you manage a random testing pool, a drug-free workplace program, or ongoing pre-employment screening, call us and we will can up a direct line of coordination rather than scheduling each collection from scratch.
